Flu Clinic Returns for Third Year on October 23

Dear Parents/Guardians:

We are very pleased to be able to offer a special program again this year that can be a huge benefit for all our Coloma families!! 

Health Heroes will be coming to Coloma Community Schools on Monday, October 23 to offer FREE flu vaccinations to all our students.  They will provide the IIV-4 Flu Injection for any student that chooses to be vaccinated.  In order to have your child vaccinated, parents/ guardians must fill out and sign a consent form.  Consent forms will be sent home with our K-5 students, emailed to families with an email address on file, and can also be obtained at each school office.  Both an English and Spanish version of the forms are attached below.  

It is important to fill out ALL sections on the consent form COMPLETELY and have your child return it to their teacher or their school office as soon as possible, but no later than Friday, October 20.  If your child has health insurance or Medicaid, please fill that section out with the Insurance Company Name, the Cardholder’s Name and Date of Birth, as well as the Contract ID.  If your child is uninsured or underinsured, please be sure to check the appropriate box on the consent form.

Money collected from the insurance company helps fund this program.  Regardless of your insurance coverage or what your insurance company pays, there is NO COST to you for the vaccine. Again, the date for our flu clinic is Monday, October 23, so please send your forms in as soon as possible.
